That happened when I added FMA 18 to the db (and also to my mylist):

ERROR:
Can't call method "prepare" on an undefined value at /var/www/anidb/perl-bin/modules/adb_db.pm line 455.

It was added to the db, but not to my mylist.
Then this happened:

this was not related to the db entry.

for reasons beyond me the db server restarted itself and disconnected all pending connections.
this resulted in some db connection problems.
cgi -> error page
chii -> restart
